Payment is not contingent on your performance on the test, and no refunds will be issued based on the outcome of any test. Please note that the registration fee you are paying reserves your spot for the specified assessment date. Spots are limited and therefore, we do not issue refunds once your registration is submitted to this website. As a candidate, you are responsible for understanding these terms. CHIP, LLC is offering a non-tangible irrevocable service.
Please note that we do not satisfy any refund requests based on the outcome or results of your test. If you fail to appear at your assigned test date no refund will be issued. Additionally, CHIP, LLC reserves the right to change the advertised test venue or date for unforeseen events or matters out of our control.
Please note that on the day of the test you must present a signed, stamped and dated Medical Approval Form. If you appear at the test site without the completed Medical Approval Form you will not be permitted to test and no refund will be issued. You must also present two forms of identification when you arrive to test. One form of identification must be a government issued ID. If you fail to bring proper identification you will not be permitted to test and no refund will be issued.
Your Agreement with CHIP, LLC
By clicking “Confirm & Submit” on the registration form, I certify that all statements made on or in registration form are true, complete and correct to the best of my knowledge and belief and are made in good faith. I also certify and understand that I am required to bring a signed, stamped and dated Medical Approval Form and two forms of identification on the day of the test.
Consistent with the United States Electronic Signatures in Global and National Commerce Act, I understand that clicking “Confirm & Submit” constitutes my digital signature which, if this were a paper application, I would be signing with pen.

Is it free?
Although all SMS Alerts are complimentary, Message & Data rates may apply. Depending on your text plan, you may be charged by your carrier. By enrolling in the Service, you certify that you are over 18 years of age and (a) you are the account holder or (b) you have the account holder's permission to do so.
What if I don't want to receive any more SMS messages?
To stop just your SMS Alerts, simply login to your account and adjust your communication settings.

FURTHERMORE, WE ARE NOT RESPONSIBLE FOR CIRCUMSTANCES BEYOND OUR CONTROL, INCLUDING, WITHOUT LIMITATION, ACTS OR OMISSIONS OF OTHERS, ATMOSPHERIC CONDITIONS OR ACTS OF GOD. WE DO NOT PROMISE ERROR-FREE SERVICE. WE WILL NOT BE LIABLE FOR ANY DELAYS IN THE RECEIPT OF ANY SMS MESSAGES AS DELIVERY IS SUBJECT TO THE EFFECTIVE TRANSMISSION FROM YOUR NETWORK OPERATOR.
Limitation of Liability
We are not responsible and will not be liable for any other damages of any nature, including any incidental, special or consequential damages (such as lost profits or lost business opportunities), punitive damages or attorney's fees.
